Abstract
Description
п.The invention relates to medicine, specifically to dressings used to close and treat burns, wounds of various etiologies, trophic ulcers, bedsores, etc.
The effectiveness of dressings designed to close and treat various traumatic injuries, wounds and burns is determined mainly by their ability to prevent the development of infection and to absorb wound exudate. In addition, the therapeutic effectiveness of the dressing increases if it provides other types of therapeutic effects (analgesia, prevention of inflammation and accelerates healing).
Недостатком этой повязки является малая сорбция (не более 2 г/г), относительно низкая антимикробная активность и отсутствие пролонгированного лечебного действия (смена повязок производится 2 раза в сутки).Known medical dressing Inadine (Inadine, catalog Systagenix Wound Management, manufactured by Johnson & Johnson, USA), consisting of a woven viscose fabric soaked in a solution of povidone iodine. The disadvantage of this dressing is a small sorption (not more than 2 g / g), a relatively low antimicrobial activity and the absence of a prolonged therapeutic effect (dressings are changed 2 times a day).
These shortcomings are partially eliminated in the Activetex dressing adopted as a prototype with a prolonged therapeutic effect, which uses antiseptics chlorhexidine and / or furagin (RF Patent No. 2101033, class A61L 15/22, 1996). "Activtex" has a higher sorption ability - up to 4.5 g / g and a prolonged therapeutic effect - up to 72 hours. However, these antiseptics have a limited spectrum of antimicrobial activity, and the sorption capacity of this dressing is insufficient, especially for purulent wounds and burns in the hydration stage.
An object of the invention is a bandage with a prolonged therapeutic effect for the treatment of burns and wounds of various etiologies with an expanded spectrum of antimicrobial activity, increased antimicrobial activity and with increased sorption ability.
The problem is solved by a three-component design of the dressing, consisting of a treatment layer, a sorption layer and an adhesive fixing frame located between them. The design of the dressing is illustrated in the drawing. In the claimed dressing, butol is used as an antiseptic, which was not previously used in dressings (RF Patent No. 2154148, class A61K 31/47, 31/71, 1998) with a content in the treatment layer from 0.05 to 1.0 mg / cm 2 . When the butanol content is less than 0.05 mg / cm 2 , the antimicrobial activity of the dressing is significantly reduced, and when the butanol content is greater than 1.0 mg / cm 2 , the likelihood of allergic reactions increases.
The drug carriers of the treatment layer facing the wound (1 - in the drawing) are: cotton knitted fabric weighing 110-210 g / m 2 of special knitting with holes, non-woven viscose-polyester fabric with holes with a diameter of 0.4-2.4 mm and weight 60- 100 g / m 2 or textile cotton mesh weighing 140-220 g / m 2 with holes of at least 1.2 × 1.2 mm in size. A prolonged therapeutic effect and atraumaticity are provided by the use of aqueous solutions of an antiseptic and biocompatible polysaccharide polymers: alginates and carboxymethyl cellulose.
В лечебную композицию могут быть дополнительно включены лекарственные препараты гемостатического (например, глюконат железа, капрофер, аминокапроновая кислота), анестезирующего (например, лидокаин, дикаин, анилокаин или тримекаин), антиоксидантного (например ά-токоферол, дигидрокверцитин или рутин) или ранозаживляющего действия (например, масло эвкалипта, масло облепихи, гидроксиапатит и хондроитин сульфат), обеспечивающие дополнительное лечебное действие заявленной повязкиMedicinal preparations of hemostatic (e.g., iron gluconate, caprofer, aminocaproic acid), anesthetizing (e.g., lidocaine, dicaine, anilocaine, or trimecaine), antioxidant (e.g. ά-tocopherol, dihydroquercytin, or rutin) or wound healing effects (can be additionally included in the treatment composition). for example, eucalyptus oil, sea buckthorn oil, hydroxyapatite and chondroitin sulfate), providing additional therapeutic effect of the claimed dressing
The increased sorption ability is ensured by the fact that on top of the treatment layer facing the wound, there is a removable sorption layer (3 in the drawing) made of needle-punched non-woven cotton viscose fabric, which provides an increase in the sorption ability of the dressing to 15 g / g. An additional increase in sorption ability with prolonged use of the dressing is achieved by replacing the sorption layer.
For this purpose, an adhesive frame (2 - in the drawing) is located between the treatment and sorption layers, which holds the sorption layer and ensures its fixation during the change, made, for example, of non-woven fabric with a thickness of 0.1-0.5 mm and a width of 6-10 mm. A layer of adhesive, for example, medical glue used in the manufacture of medical plasters, is applied on both sides of the frame. The choice of the thickness of the adhesive frame is determined by the fact that webs with a thickness of less than 0.1 mm do not have the proper mechanical strength, and when the thickness of the nonwoven fabric exceeds 0.5 mm, the contact between the treatment and sorption layers worsens, which leads to a decrease in the sorption ability of the dressing. With a frame width of less than 6.0 mm, the reliability of fixing the sorption layer is reduced, and with a frame width of more than 10 mm its useful area is reduced.
The presence of an effective antiseptic in the proposed medical dressing prevents the development of infection and thereby helps to accelerate the healing of burns and wounds. The openings in the treatment layer provide a free outflow of exudate, while the sorption layer can be replaced on demand without damaging the wound surface. The proposed dressing has a high degree of surface modeling of wounds, is convenient to use. The test results of the proposed medical dressing in comparison with the analogue (Inadine) and the prototype (antimicrobial cloth from a knitted fabric, sterile "Activetex-HF" with chlorhexidine and furagin) are shown in Table 1. The data on the sorption ability of the claimed dressing presented in Table 1 correspond to its use without changing the sorption layer. Each change in the sorption layer increases the sorption ability of the dressing by at least 10 g / g.
Thus, the dressing according to this application has a higher antimicrobial activity and sorption ability.
