Abstract
La présente invention a pour objet un dispositif de protection d’une surface prédéterminée (9) apte à empêcher celle-ci de supporter toutes contraintes extérieures, tel qu’il comporte : - une première pièce (8) rigide, appelée concentrateur, apte à supporter les dites contraintes extérieures et dont la surface de projection sur la surface (9), perpendiculairement à celle-ci, au moins égale à la dite surface (9) à protéger, - d’au moins une deuxième pièce (10) ayant une partie déformable et appelée transmetteur, supportant et bloquant au moins radialement le concentrateur (8), et ledit transmetteur recevant les forces résultant des contraintes exercées sur le concentrateur (8) maintient celui-ci à une hauteur minimum (h) au-dessus de ladite surface à protéger (9), lorsque le concentrateur (8) lui transmet lesdites forces, et - d’au moins une troisième pièce (11), appelée effecteur, apte à être disposée contre une surface d'appui (3) dite externe, entourant au moins la surface (9) à protéger sans la recouvrir, et apte à prendre en charge les forces que le concentrateur (8) lui transmet par le transmetteur (10) pour les répartir en pression exercée sur ladite surface d’appui (3) externe. Il n'est donc pas surprenant de trouver de nombreux brevets de dispositifs anti-escarre reposant sur l’évidement des supports au droit des zones à risques d’escarres afin d’éliminer la charge : on peut citer dans ce domaine la demande de brevet PCT WO2018/038402 de la société coréenne One For All enseignant un support de type matelas, pour un patient allongé afin de prévenir des ulcères décubitus, qui constituent un cas très fréquent d’escarres rapporté dans la littérature brevets pour les personnes devant rester immobiles en position allongée ; le support décrit dans cette demande de brevet PCT est composé d’une pluralité de couches de rembourrage comportant un mécanisme de distribution de pression corporelle et des trous traversant formés dans chaque couche de rembourrage, la taille du trou traversant formé dans une couche de rembourrage supérieure étant plus petite que la taille du trou traversant la couche de rembourrage inférieure adjacente de manière à être en communication l'un avec l'autre.The majority of solutions offered are supports – mattresses, cushions, wedges – made from structured or raw materials designed to reduce pressure. - pour les escarres du talon et du pied en général, ainsi que pour la protection des plaies sur la voute plantaire, il existe des dispositifs adaptés, qui font encore l’objet de nombreux brevets déposés tel que la demande de brevet chinois CN211932856 publié en 2020 par l’Hopital Zhongnan de l’Université de Wuhan qui décrit un coussin de décompression maintenu sur le pied par un manchon d'orteil et une bande de fixation entourant la cheville, le corps de ce coussin de décompression étant relié également de manière fixe à un coussin de talon et comprenant au moins une ouverture à l’endroit de la plaie, un coussin en éponge pouvant être mis dans la cavité avec du gel de silice afin d’apporter un certain confort au patient,
- pour les escarres du sacrum, il a été spécifiquement développé une cale de décubitus, cylindre en mousse à section triangulaire de la longueur du matelas qui permet de caler le patient en position latérale à 45°, puis de l’autre côté toujours à 45° après changement de position par les personnels soignants. Une tendance relevée dans l’analyse des dépôts de brevets récents relatifs aux matelas anti-escarres (voir exemple du brevet PCT cité précédemment) est de réaliser une décharge centrale plus prononcée obtenue en par évidement simple ou multiple, par emploi de mousse mono ou multicouche de moindre résistance à la compression – ce qui pose encore le problème du bon positionnement du patient – morphologie, taille, compatibilité avec position assise, glissé avant, …Moreover :
